Three Connecticut skilled nursing and rehabilitation centers operated by National Health Care Associates, Inc. (NHCA) have joined UConn Health's Preferred Post-Acute Care Network, expanding the network's capacity to support hospital-to-rehabilitation transitions across the state.
The designated centers are Maple View Health and Rehabilitation Center in Rocky Hill, The Hebrew Center for Health and Rehabilitation in West Hartford, and The Pines at Bristol Center for Health and Rehabilitation in Bristol. Membership in the network is extended to post-acute care providers that meet standards in clinical outcomes, quality measures, patient satisfaction, and care coordination.

As network members, the three centers will coordinate with UConn Health providers on individualized care planning and continuity of care throughout the rehabilitation process.
