Connecticut Children’s Care Network Partners with Innovaccer to Advance Pediatric Care

Connecticut Children’s Care Network has entered into a strategic partnership with Innovaccer Inc., a healthcare AI company, to strengthen population health management and improve quality outcomes for more than 140,000 pediatric patients across the state.

 

The Network, which includes more than 1,450 providers at 44 practice sites, will implement Innovaccer’s Data Activation Platform, Population Health Analytics, and Quality Reporting solutions to improve care coordination, reporting, and outcomes. These tools will integrate claims and clinical data, providing advanced analytics, physician performance insights, and supplemental data reporting to support value-based care contracts.
